Address Change Requirements

Documents Needed to Update or Change Address

If lease or own home 3 proof of residency documents are required.

ONE of the following documents:

• Current Property Tax Bill
• Current Mortgage Statement
• Property Transfer Affidavit
• Warranty Deed
• Current Lease (signed and dated)

TWO additional documents from the following list (all must include name, address, and date of issue within past 60 days):

• ONE must be a Utility Bill (DTE or Consumers)
• ONE must be from the following list:
  • IRS Information
  • Charge card statement
  • Bank Statement
  • Employment record
  • Health Insurance document
  • Renter's or homeowner's insurance
