You can register your child at the school
that services your neighborhood. Please call the
school to arrange an appointment to process the
paperwork. You will need the following
documents:
- Birth Certificate - Michigan law requires
that the school maintain a copy of each
student's birth certificate. Parents
must submit a certified birth certificate
(original raised seal). If the birth
certificate is not in English, a passport or
VISA must also be submitted.
- Immunization records showing specific dates
for each immunization.
- Proof of residency
- The parent/guardian
must show the following which will verify the
family and student reside in the Northville
Public School District:
- Signed Lease or Mortgage/Tax Statement
- Residential Affidavit (notarized and
dated)
- Property Transfer Affidavit
- Warranty Deed
- Plus two (2) additional proofs. Office
personnel will make copies:
- IRS Information
- Charge card statement (Financial
Information & credit card number may be
crossed out)
- Bank Statement
- Utility Bill
- Employment record
- Auto/Health Insurance document
- Copies of your child’s most current IEP, MET
and any related reports.
Please complete and sign a Release of Records
form to obtain general and special education
information from your child’s previous school.
